Tdap Requirement for Rising 6th Grade Students!
The Tdap (tetanus, diphtheria and pertussis) vaccination is a State of Virginia mandatory immunization for all rising 6th grade students. If a student is exempted from this requirement a parent must provide written documentation from a doctor indicating:

     • a medical exemption

     • child is under age 11 and an immunization vaccine will not be given at this time

     • or documentation to present that student has had a vaccine in past 5 years

All students who do not have documentation of all immunizations or a letter from a physician will not be permitted to attend school and will not get credit for the day.  Local health departments will provide the Tdap vaccine to uninsured rising 6th grade students (some fees may apply). For health department information in the Woodbridge area, call 703 792-7300 or contact your private physician.
